Doctor listing or practice listing
A named provider sometimes earns their own profile. A solo branded practice usually should not split reviews across two listings at the same address. A second listing is how you invite a suspension. See suspended profiles.
Practice profile
The default. NAP matches the site. Services match the money pages. Reviews stay in one pile.
Practitioner profile
When patients search the doctor's name, and that person is a real, distinct public entity. Not a workaround to rank a second city.
Website city pages are a different job: city pages. Do not invent a second GBP to match a thin city URL.
