The real search - not the pitch
Cash-pay hormone practice, without the Instagram-first order
Most "start a BHRT clinic" pages are selling a course. This one maps the phrases providers actually search, the three models those phrases point at, and the work that has to exist before anyone spends a dollar on ads.
Key facts
- Search language is generic first: cash-based practice, DPC, concierge, Medicare opt-out. Hormone-specific queries come second and are dominated by vendors.
- Cash-pay, DPC, and concierge are not synonyms. DPC is usually a membership for primary care. Concierge often keeps some insurance. A cash HRT line can sit inside any of those, or inside a med spa, OB/GYN, or urology practice.
- Federal paperwork that actually exists: Medicare opt-out if you will see Medicare patients without billing Medicare.
- Demand is not theoretical. 4,000+ inquiries on the forms since 2021. 300+ practices on the system. 600+ into a clinic that did not exist a year ago.
A working order that does not start with ads
Then, and only then, demand systems.
Scope
Menopause, testosterone, thyroid, peptides, or some mix. If you cannot explain the offer out loud, you are not ready to advertise it.
Payer posture
No Medicare patients, enrolled, or opted out. That is an affidavit and a contract, or it is not. Not a branding exercise.
Prescribing and monitoring
Testosterone is controlled. Compounded hormones and in-office procedures have state and federal constraints this page will not flatten. Get counsel.
Then demand
Site, Google Business Profile, rack card, EvexiPEL as the anchor. Paid ads on a thin clinical story is how cash-pay HRT got a reputation problem.
Questions
Should I call it DPC if I only offer HRT?
Usually no. DPC is a primary-care membership model. A cash HRT service line is allowed to be exactly that. Borrowing the DPC label is how you inherit rules you did not mean to take on.
Where does MarketMedical start?
After you know the medicine and the payer posture. I handle the demand side: EvexiPEL, exosomes, peptides, the website, the card at the front desk. I do not write your protocols.
When you are ready for demand
Strategy call. EvexiPEL first if that is the line. Website and rack cards when the offer is real.
One call. Real numbers. No pressure.
